If you are a new individual therapy or assessment patient, please select the "Patient Registration Packet" under New Patient Forms.


If you are seeking a bariatric or spinal cord stimulator psychological evaluation, you will need to complete the packet labeled "Pre-Surgical Evaluation" under New Patient Forms.


If you are interested in biofeedback therapy, please select the "Biofeedback Therapy" packet under New Patient Forms.


All new patient forms are required to be completed prior to the first appointment.


All new patients must provide a valid ID and insurance card (except Tricare, VA Community Care, and EAPs who only need to provide a valid ID) when they complete the form packet. Please be sure to have pictures of the front and back of these cards before you begin your forms.
