The military and our Veterans are an extensive part of the surrounding Jacksonville community in which our providers live and practice. Our providers take pride in being able to give back to these individuals, families and couples. Each of our providers has a special connection to the military and to Veterans as they are a product of these unique family units themselves and have experienced the commitment, dedication, and sacrifice that comes with these roles. 


Our providers are fortunate to be able to help military families and veterans through Tricare, veterans through the VA Community Care program and active duty military through Military One Source. 



*If you are filing a VA disability claim and need a DBQ completed, you will need to make an appointment with our provider, Dr. Milne or Ms. Miller depending on the type of benefit increase you are seeking. Please navigate to our Assessment/Evaluations page for information and forms. Please note that insurance does not pay for the completion of the DBQ and you will need to pay for this out of pocket.
